Developed June 21, 2016 by Barrington H. Brennen and a team of editors and approved by the administration of the South Bahamas Conference

READ FIRST

EVALUATION:  If you did or did not attend Camp Meeting 2016 or any other Camp Meeting in the South Bahamas Conference of Seventh-day Adventist, would you kindly fill this evaluation form to help us plan even greater Camp Meetings that can meet your needs and expectations.

CONFIDENTIALITY:  Note that you are not being asked to identify yourself and it is impossible for the designers of the survey to know who are responding to the questions.  Answer as many questions as possible. Feel free to make comments, give suggestions or ask your questions in the boxes provided in the survey.

THE ENTIRE FAMILY ON ONE COMPUTER:  If there is one computer in the home you are still able to have multiple persons use the same computer to do the evaluation. When one person has completed the survey and have clicked "Submit," then go back to the survey and another person can do it again. This can be done several times on one computer or from one IP address. This will allow for multiple persons in one home with one computer to complete the survey. This adjustment was made just accommodate this survey.  Thanks to SPARKLIT mini surveys.

OPTIONAL QUESTIONS:  Each question is optional, although everyone is being asked to complete the first four questions.   If a question is not relevant to you just simply ignore it and move on to the next question.

SEND TO FRIENDS & RELATIVES: You are encouraged to email the link to this evaluation survey to all your Adventist friends or relatives in The Bahamas or even those overseas who have attended our Camp Meetings. We would like at least 1000 respondents to this evaluation.
